By using this form, you consent to electronic communications with our office, understanding no medical information may be transmitted by this portal, and no medical questions will be answered. You cannot use this form to request an appointment; you cannot just enter whatever date and time you want below. It is only for KNOWN appointments.
NB: If you cancel within 72 hours, you are subject to a $100 cancellation fee to be paid before any future visit.
